MISSION STATEMENT Enrollment Form
The Booster Club is being established for support and encouragement through parent participation and to raise funds to enhance and expand the Academic, Social and Athletic programs that are available to each student at Shining Light Academy.
Joining the Booster Club and volunteering at school events are great ways to support our students, to meet other parents, and to get involved. This also enables you to attend our monthly meetings.
Your Membership will help provide financial support for:
· Facility Improvements
· Athletic Equipment
· Team Uniforms
· Supplement the needs of the Athletic
Department, Social and Academic clubs.
· Encourages a positive environment through
parent and volunteer participation
The Saturday morning basketball league will also be supported by the Booster Club.
Individual & Family Passes will be available to purchase at all home games.
Fundraisers will include 50/50 raffles, concessions, sponsorships, golf tournament, merchandise sales and many more.

If you would like to become a member, please print and complete the enrollment form and return it along with the $10 dues to your child’s homeroom teacher.
The next Booster Club Meeting will be held at ----------, 2009 in the SLA cafeteria. This meeting is for all members and anyone that would like to join the Booster Club!
